We are accepting new patients. An appointment is usually available within 2 weeks on an average and earlier if necessary.

When You Call ...

Make sure you give the following information

• Name
• Phone number
• Insurance information
• Reason for visit.

We will call and remind you the previous evening about your upcoming appointment using our computerized system.

New Patients

• Fill out the patient info forms - available in this website (click here to download)
• Arrive 30 minutes before scheduled appointment time.
• Bring your insurance card - if unavailable please reschedule appointment.
• Bring ALL your medication bottles - just list is not enough.

Cancellation Policy

·         Appointments must be cancelled by calling at least 48 hours before visit.

·         If you do have to cancel, please reschedule at the same time or as soon as possible after that. 

·         Same day cancellations are treated as  "no-shows"  and we reserve the right to bill for this. The NO-SHOW FEE is $25.

Medication Refill Policy

·         Prescriptions are given after evaluation, review of patients health/labs at each visit. 

·         If any patient should run out of prescription, please do not wait until the last day, as we need 3 days to call in refills. We will call in refills as long as patient has been regular with their appointments.

·         Anyone running out of prescriptions due to missed/rescheduled appointments may be charged a fee for calling in the prescription and that depends on the number of prescriptions. Appointments have to be made at that call and we may call up to a maximum of 15 days of medications.
