We are accepting new patients. An appointment is usually available within 2 weeks on an average and earlier if necessary.

When You Call ...

Make sure you give the following information

• Name
• Phone number
• Insurance information
• Reason for visit.
  
  

We will call and remind you the previous evening about your upcoming appointment using our computerized system.

Cancellation Policy

Appointments must be cancelled by calling at least 48 hours before visit, if not it will be treated as a no-show and we reserve the right to bill for this.The NO-SHOW FEE is $25.

New Patients

We request the following from new patients

• Fill out the patient history form - available in this website (click here to download)
• Arrive 30 minutes before scheduled appointment time.
• Bring your insurance card - if unavailable please reschedule appointment.
• Bring any previous medical records if available.
• Bring ALL your medication BOTTLES (just a LIST is not enough).
