Thank you for choosing Dr. Raya for your health care needs. Dr.
Raya is committed to the improved health of our patients by providing
appropriate, high quality, comprehensive family health care.
We are accepting new patients. An appointment is usually available within 2 weeks
on average and earlier if necessary.

We will call and remind you
the previous evening about your upcoming appointment using our computerized
system.
CANCELLATION POLICY
·
Appointments must be cancelled
by calling at least 48 hours before visit.
·
If you do have to
cancel, please reschedule at the same time or as soon as possible after that.
·
Same day cancellations
are treated as "no-shows" and we reserve the right to bill for this. The NO-SHOW FEE is $25.
·
Prescriptions are given
after evaluation, review of patients health/labs at each visit.
·
If any patient should
run out of prescription, please do not wait until the last day, as we need 3 days
to call in refills. We will call in refills as long as patient has been regular
with their appointments.
·
Anyone running out of
prescriptions due to missed/rescheduled appointments may be charged a fee for
calling in the prescription and that depends on the number of prescriptions.

Appointments have to be
made at that call and we may call up to a maximum of 15 days of medications.
